Based on observations the facility failed to maintain there sprinkler system in accordance with NFPA 13.

Findings Include:

While inspecting the sprinkler system on 01/12/10 at 2:00p.m., the surveyor observed 38 sprinkler heads located throughout the facility (corridors, offices, radiology, laundry, kitchen, and sterile supply) covered with dirt, cobwebs, and grease.

This deficient has the potential of affecting the sprinkler system from dispersing water upon activation. The administrator and the maintenance director were notified during the investigation as well as in the exit conference.
