Based on visual observation the facility failed to maintain the separation of hazardous areas from other parts of the building, including the egress corridor. Hazardous areas are required to be constructed to resist the passage of smoke. The deficient practice had the potential to affect 10 of 10 residents.
2 of 5 smoke compartments have hazardous areas that are not separated.

Findings:

During the facility tour on 5/27/2016 observation revealed doors to storage rooms on halls B & C > 50 sq ft were not self closing.

Interview with maintenance revealed the facility was not aware the doors of the hazardous areas were not self closing.
