EVALUATION INCLUDES INVENTORY OF ASSETS

Based on record review and staff interview, the facility failed to ensure that psychiatric evaluations included an inventory of specific patient assets that could be used in treatment planning for 8 of 8 sample patients (1, 2, 3, 4, 5, 6, 7, and 8). The failure to identify patient assets can impair the treatment team's ability to develop treatment interventions utilizing the individual strengths of each patient.

Findings include:

A. Record Review

The admission Psychiatric Evaluations (dates in parentheses) for the following patients did not contain specific patient assets:

1. Patient 1 (10/16/10). The Psychiatric Evaluation listed the non-specific assets: "She has caring family members and healthcare providers."

2. Patient 2 (10/22/10). The Psychiatric Evaluation listed the non-specific assets: "She is attractive. She is willing to get help presently. She has caring family members and healthcare providers."

3. Patient 3 (10/21/10). The Psychiatric Evaluation listed the non-specific assets: "She is reasonably attractive. She has caring family members and healthcare providers."

4. Patient 4 (10/20/10). The Psychiatric Evaluation listed the non-specific assets: "He has caring family members and healthcare providers. He is willing to get help presently."

5. Patient 5 (10/21/10). The Psychiatric Evaluation listed the non-specific assets: "She has caring family members and healthcare providers. She is willing to get help presently."

6. Patient 6 (10/12/10). The Psychiatric Evaluation listed the non-specific assets: "He is attractive. He has caring family members and healthcare providers."

7. Patient 7 (10/12/10). The Psychiatric Evaluation listed the non-specific assets: "He is agreeable at times. He has caring family members and healthcare providers. He is willing to get help presently."

8. Patient 8 (10/20/10). The Psychiatric Evaluation listed the non-specific assets: "He is reasonably attractive. He has caring family members and healthcare providers."

B. Staff Interview

During an interview with the Medical Director (who was also the attending psychiatrist for these patients) on 10/27/10 at 8:00a.m., he acknowledged that the assets identified on these psychiatric assessments were not specific patient strengths that could be utilized for treatment planning.

PLAN BASED ON INVENTORY OF STRENGTHS/DISABILITIES

Based on record review and interviews, it was determined that the facility failed to develop and document treatment plans that included an inventory of individualized strengths and disabilities for use in treatment planning for 8 of 8 sample patients (1, 2, 3, 4, 5, 6, 7, and 8). This failure potentially diminishes the effectiveness of treatment by not utilizing the specific individual strengths of each patient in their treatment, with an awareness of their disabilities.

Findings include:

A. Record Review

For the following treatment plans (dates in parentheses), no inventory of strengths and disabilities were included: Patient 1 (10/16/10), Patient 2 (10/22/10), Patient 3 (10/21/10), Patient 4 (10/20/10), Patient 5 (10/21/10), Patient 6 (10/12/10), Patient 7 (10/12/10), and Patient 8 (10/20/10).

B. Interviews

During an interview with the Director of Nursing, Assistant Director of Nursing, Director of Social Services/Activities, and Chief Executive Officer on 10/27/10 at 11:00a.m., they acknowledged that these treatment plans did not contain an inventory of specific strengths and disabilities that were identified to be utilized in treatment and treatment planning.
