Based on visual observation the facility failed to provide corridor doors that were not closing and latching in the frame. When the doors latch a smoke resistive seal is formed to protect the room ' s occupants. The deficient practice had the potential to affect 6 of 6 residents.
1 of 2 corridors had doors that were deficient.
Findings:
During the facility tour on 9/1/2015 observation revealed the doors to room 102 & 107 did not latch in the frame.
Interview with Maintenance revealed the facility was not aware of these doors to Room not latching in the frame.

Based on visual observation the facility failed to maintain the separation of hazardous areas from other parts of the building, including the egress corridor. Hazardous areas are required to be constructed to resist the passage of smoke. The deficient practice had the potential to affect 6 of 6 residents.
1 of 2 smoke compartments have hazardous areas that are not separated.
Findings:
During the facility tour on 9/1/2015 observation revealed 13 rooms now being used for the storage of combustible materials, these rooms are greater than 50 sq ft. Non of the doors to these rooms are self closing. Three of these rooms: Patient storage, Medical Records and Rm 302 would not latch in the frame.
Interview with Maintenance revealed the facility was not aware that the doors to the hazardous areas were required to self-close and latch in the frame.
