Based on document review and interview the facility failed to provide the patients with a phone number and address to file a grievance with the state agency; resulting in the potential of denying all patients the the right to file a grievance with an outside agency.
Findings include:

On 1/6/15 at 1400 during review of the patient rights documentation, provided to patients upon admission to the facility, it was revealed that the documentation failed to list an address and phone number of the State of Michigan to whom which patients or a patients' representative can contact to file a complaint/grievance.

On 1/7/15 at 0900 during an interview with staff A, the Director of Quality, when asked if this was all the information given to patients on how and with whom to file a complaint/grievance with, she replied, "Yes."
