Based on visual observation the facility failed to maintain the separation of hazardous areas from other parts of the building, including the egress corridor. Hazardous areas are required to be constructed to resist the passage of smoke. The deficient practice had the potential to affect 3 of 3 residents of the facility.
Findings:
During the facility tour on 1/28/2011, between the hours of 8:30am and 11:30am, the doors to the wheelchair storage room and the first aid storage room did not have self-closing devices causing the doors and rooms to not be smoke resistive.

Based on visual observation the facility failed to maintain sprinkler system devices through out the facility.. The deficient practice had the potential to affect 2 of the 2 smoke compartments and could potentially affect 3 of the 3 residents.
Findings:
1) - During facility tour on 01/28/2011 between the hours of 8:30am and 11:30am, sprinkler heads in rooms #1, #2, #7, #8, #9, and in the front office area above the copier have paint that would affect the activation of the sprinklers in the case of a fire emergency.
2) - During facility tour on 01/28/2011 between the hours of 8:30am and 11:30am, sprinkler piping in the medical records room had wires that are attached and supporting the pipes.

Based on visual observation the facility failed to provide proper ashtray containers. The deficient practice had the potential to allow smoke and fire back into the facilities corridors and means of egress and affect approximately 3 of the 3 residents of the facility.
Findings:
During facility tour on 01/28/2011 between the hours of 8:30am and 11:30am, the ashtray containers in the smoking areas were not of the metal type with self-closing covers. The type that were present had a plastic bases with a plastic tube with no self-closing lids.

Based on visual observation , the facility failed to assure that the refrigerator used to store the medication for the residents was tied into the emergency power.. In cases of a power outage the emergency generator powers essential life safety equipment for the facility. The deficient practice had the potential to affect 3 of the 3 residents of the facility.
Findings:
During the facility tour on 01/28/2011, between the hours of 8:30am and 11:30am, the emergency generator was tested and during the walk through of the facility, the nurses station refrigerator that stores the medication for the residents of the facility was not running or tied into a red emergency plug.
