Based on record review and interview, the patient was not provided care in a safe setting in that the House Supervisor, a registered nurse, did not move the licensed vocational nurse (LVN) to another unit when the LVN became confrontational with the patient.
Findings Included:
The facility reported that the LVN (Employee #4) became confrontational with a patient who was "taunting" patients and the LVN (Employee #4). The House Supervisor (Employee #3) did not intervene and move the nurse to another unit. The LVN began to exchange words with the patient.
The medical record for patient #1 included that on 03/15/11 at 10:30 PM the patient was instigating other patients and was verbally aggressive. She jumped across the nurses' station counter and began threatening staff. The patient was placed in a physical hold and medicated with emergency medications.
In an interview with the surveyor at 4:00 PM on 06/28/11 the Director of Clinical Services (Employee #2) was asked if the House Supervisor was available to the unit. She stated the House Supervisor was on the unit during the patient's behavioral escalation. She also reported that the event was witnessed by numerous staff and patients.
