Based on record review and interview Hospital A did not provide treatment to 1 of 1 patient (patient #4) who ran away on 9/18/14 from Hospital A following a mental health assessment. Patient #4 was a 17 year old male patient with a diagnosis of moderate mental retardation (MR) who ran away on two separate occasions after Patient #4's mother presented to Hospital A to take the patient home or to another treatment site. Patient #4 was later taken to Hospital B where he received treatment and was discharged to home on 9/19/14.

Findings Included:
1) Patient #4 arrived at Hospital A on 9/18/14 after he ran away from a therapist office across the street from Hospital A. Patient #4 was well known to Hospital A and had a diagnosis of Moderate Mental Retardation (MR).
2) Patient #4 had a mental health assessment at Hospital A. Physician #6 reported patient #4 did not meet the admission criteria for in-patient care at Hospital A. The physician recommended out-patient care and discharged the patient to home.
3) No treatment was initiated prior to Patient #4's release.
4) Patient #4 was taken to Hospital B and was discharged to home within 24 hours.

Based on record review and interview Hospital A did not provide treatment to 1 of 1 patient (patient #4) who ran away from Hospital A following a mental health assessment on 9/18/14. The patient was a 17 year old male with a diagnosis of moderate mental retardation (MR) who ran away on two separate occasions when his mother presented to Hospital A to take the patient home or to another treatment site.
Patient #4 was later taken to Hospital B and received treatment and was discharged to home on 9/19/14.

Findings included:

1) On 9/18/14, Patient #4 presented to Hospital A after he ran away from a therapist office across the street from Hospital A. He was well known to Hospital A and had a diagnosis of moderate mental retardation (MR). When patient #4's mother arrived at Hospital A from the therapist office the patient ran away from the waiting area of Hospital A. The mother stated this was a learned behavior and requested assistance from the police. An APOWW (Apprehension by a Police Officer Without a Warrant) was completed and patient #4 was escorted back to a locked unit at Hospital A. The mother stated she did not want in-patient care but a long term residential center for patient #4.
2) Patient #4 was taken to a locked unit and a mental health assessment was completed. The assessment included on must questions that the patient could not or would not respond due to his MR. The mother was not present for the assessment. The physician determined the patient did not meet the hospital criteria for inpatient mental health treatment and should be referred to an alternative level of care - outpatient care. No other type of treatment was initiated.
3) When the mother left the locked unit with patient #4, the patient once again ran away from Hospital A. A police officer on the campus saw the incident, called for backup and was able to stop the patient who was by that time off campus. A second APOWW was completed for patient #4. He was taken to Hospital B, a private psychiatric facility, where he was administered a cocktail (Haldol, Benadryl and Ativan) and released to home on 9/19/14.
4) In an interview with the surveyor at noon on 10/31/14 the facility administrative staff (staff #s 2 and 3) confirmed the treatment provided patient A was de-escalation. No restraints or medications were provided the patient. Patient #4 was calm and cooperative until his mother's arrival at Hospital A.
