Based on record review and interview, the hospital failed to set priorities for its performance improvement activities that affect health outcomes, patient safety, and quality of care and to take actions aimed at performance improvement for one of one patient (Patient #20) who was allegedly touched inappropriately by Patient #1 on 10/25/14. The alleged incident was not investigated.

Findings included:

Patient #1's nursing notes dated 10/25/14 at 20:25 reflected "Sexual assault allegation filed with staff by female...[patient]...house supervisor notified at 20:28."

Hospital Personnel #3 was interviewed on 12/10/14 at 13:40 and stated the incident was not investigated according to hospital policy because it was only Patient #1 allegedly touching Patient #20's breast. No incident report was completed. The hospital's CEO, Administrator, or Police were not notified.

Hospital Personnel #1 was interviewed on 12/10/14 at 14:50 and asked whether she had knowledge of the incident and stated, "No, not until a few minutes ago."

Incident reports dated 09/2014, 10/2014, and 11/2014 were reviewed and did not provide evidence of an alleged incident between Patient #1 and Patient #20 on 10/25/14.
