Based on document review and interview nursing staff failed to sign/date Patient Personal Effects Inventory in 1 of 10 patient MR's reviewed (Patient 1).
Findings include:
1. Facility policy titled: Admission of Patients (IN), PolicyStat ID: 12508272, last approved 10/2024, indicated under Documenation: The patient's belongs are documented and signed for on the Patient Belongings Inventory by the nurse or designated mental health staff member.
2. P1's MR lacked admitting staff and/or discharging staff signature/date/time on Patient Personal Effects Inventory.
3. In interview on 7/31/25 at approximately 9:02 am, A2 (Director Clinical Services) confirmed staff signature/date/time was missing from P1's personal effects inventory record.
