Based on interview and documentation review it was determined the Hospital failed to ensure 2 out of 7 Certified Nursing Assistant/nursing technician's had the appropriate training and/or competency and were performing within their scope of practice/ job descriptions.
Findings included:
The Hospital's job description for a certified nursing assistant (CNA) was reviewed. The listed job responsibilities did not included performance of electrocardiograms (EKGs).
The CNA was interviewed by telephone on 5/7/10 at 9:00 AM. The CNA said as a CNA at the Hospital I do not draw blood or perform Accuchecks but I can and do perform EKGs. The CNA said the nursing technicians taught me how to do EKGs and I also had taken a class in school.
The Hospital's job description for a nursing technician was reviewed. The listed job responsibilities included the performance of EKG's as assigned.
Randomly selected nursing technician's personnel and education files were reviewed. One out of six nursing technician's (NT #6) file did not include competency validation documentation related to performance of EKG's. NT #6 had started working at the Hospital in July 2007.
The Nurse Educator was interviewed in person on 5/10/10 at 9:00 AM. The Nurse Educator said documentation of EKG competency validation could not be located for NT #6.
