Based on observation and interview the hospital failed to ensure methods were in place to prevent and/or control the transmission of infections in that: (1) The kitchen was not maintained in a manner to ensure cleanability and prevent contamination. (2) There were no test strips available to test the sanitation range in the 3 compartment sink sanitizer solution.

Findings included:

During a tour in the kitchen on 7/15/2020 at 10:40 AM with Personnel #4 she said the dishwasher was removed 2-3 weeks ago, she wasn't sure about the date. She said since the dishwasher had been removed, the meals were served on paper plates, plastic ware, and Styrofoam cups. The pots, pans, and utensils were sanitized using the 3 sink compartment method. Personnel #4 demonstrated how the first sink was filled with dishwashing detergent, and to the third sink metered Oasis 146 Multi-Quat Sanitizer was added to water. The directions for the Multi-Quat Sanitizer posted on the wall at the sinks indicated the sanitation range in the solution should be tested with test strips. Personnel #4 said the kitchen hadn't had test strips for the sanitizer for about 2-3 weeks. She was asked by the surveyor how she could ensure the sanitation range was within acceptable parameters. She said she couldn't without the test strips.

During the tour of the kitchen observations were made of holes and peeling areas of the wall in the janitor's closet. The floor was stained and the area at the juncture of the floor and the pipes of the sink were corroded and had rust. There were 2 ceiling tiles pushed inward in the hall between the janitor's closet and the pantry room. There was an area in the ceiling above the food prep area that appeared to have exposed air filters. Personnel #4 confirmed the findings.
