Based on record review and interview, the hospital failed to ensure documentation of patient baths for five (Patients #1-5) of ten patients.
This failed practice has the likelihood to place patients at risk for infection and reduced patient comfort.
Review of Policy #S05-G documented patients were to be bathed daily, at minimum.

Review of the medical record for Patient #1 showed no documentation the patient received or refused a bath for two days in a seven-day period.

On 01/06/21 at 10:00 AM, Staff K reviewed the medical records for Patients #1-5 and stated the following:
1. There was missed documentation to show patients had received a bath.
2. Patients were to receive a bath every day and as necessary.
3. Daily baths were important for patient comfort and contributed to a reduction in germs, especially important if the patient had wounds.
