Based on observation and interview the facility failed to designate exactly which rooms are affected by the wall mounted medical gas shutoff valves, which control the flow of oxygen, air and the vacuum to patient rooms. This deficient practice effects all patients in the four patient rooms located in the central smoke compartment. The census was 27.

Findings included:

Observation, during a tour of the facility conducted on the morning of 07/23/13 at 9:50 AM, showed two sets of wall mounted shutoff valves located in the central smoke compartment. A sign above one set of shutoff valves stated "These valves control medical gases for the North wing." The North wing comprises patient rooms 101 through 119. A sign above the other set of shutoff valves stated "These valves control medical gases for the East wing." The East wing comprises patient rooms 201 through 219. Neither sign indicated which valves would shut off the oxygen, air and vacuum to the 4 ICU (Intensive Care Units) in the central smoke compartment.

During an interview on 07/23/13 at 9:51 AM, Staff O, Materials Manager, stated that the shutoff valves for the North wing also shut off the oxygen, air and vacuum to patient rooms ICU 1 and ICU 2 while the shutoff valves for the East wing also shut off oxygen, air and vacuum to patient rooms ICU 3 and ICU 4.

Section 4-3.5.4.2 of the National Fire Protection Association (NFPA 99) states: The shutoff valves shall be labeled to reflect the rooms that are controlled by such valves. Labeling shall be kept current from initial construction through acceptance.
