Based on visual observation this sprinklered facility failed to assure that the smoke compartmental of the membrane between the egress corridors and rooms adjacent to the egress corridor, were not compromised. Repairs to assure the protection of occupants and the integrity of the means of egress are essential in case of a fire or other smoke emergency. The deficient practice had the potential to affect 13 of 13 residents.
Of smoke compartments were deficient.
Findings:
During the facility tour, between the hours of 8:00 am and 4:30 PM observation revealed unsealed penetration though out this facility in the corridor walls.

Based on visual observation the facility failed to provide corridor doors that were closing and latching in the frame. When the doors latch a smoke resistive seal is formed to protect the rooms' occupants. The deficient practice had the potential to affect 3 of residents.
Of 13 corridors had doors that were deficient.
Findings:
During the facility tour, between the hours of 8:00 am and 4:00 PM observation revealed corridor room door 439, 442 were sticking and hard to open. Door to room 428 did not latch in it's frame.

Based on visual observation the facility failed to assure the construction of the smoke barriers walls. The walls are required to be continuous and properly protected from penetrations and gaps. Unprotected penetrations would permit the movement of smoke from one compartment to the other in the facility. The deficient practice had the potential to affect 13 of 13 residents.
3 Of 4 smoke barriers were deficient.
Findings:
During the facility tour, between the hours of 8:00 am and 4:00 PM observation revealed unsealed penetration in barrier wall into rehab, barrier wall into rehab office and barrier that separate Behavioral hospital and extended care.
