Based on review of Personnel Files (PF) and interviews with staff (EMP), it was determined that the facility failed to ensure that the Director of Respiratory Care Services was a medical or osteopathic doctor.
Findings include:
Review on September 29, 2021, of Personnel File (PF2) revealed that the Director of Respiratory Services was not a medical or osteopathic doctor.
Interview with EMP1 on September 29, 2021, at 2:00 PM, confirmed there was no documented evidence that a medical or osteopathic doctor was appointed the Director of Respiratory Services.
