Exit interview with the Facilities Director on October 6, 2021, between 10:45 a.m. and 11:00 a.m., confirmed the building construction deficiency.

Exit Signage

Based on observation and interview, it was determined the facility failed to install and maintain exit signage in one location, affecting one of four floors.

Findings include:

1. Observation on October 5, 2021, at 12:39 p.m., revealed the ground floor, Mechanical Room, lacked exit signage above the exit door.

Exit interview with the Facilities Director on October 6, 2021, between 10:45 a.m. and 11:00 a.m., confirmed the exit signage deficiency.

Corridor - Doors

Based on observation and interview, it was determined the facility failed to maintain corridor openings in four locations, affecting one of two floors.

Findings include:

1. Observation on October 6, 2021, between 9:22 a.m. and 9:45 a.m., revealed the following:

a. 9:22 a.m., the first floor, Dining Lounge door required adjustment to fully latch.
b. 9:33 a.m., the first floor, Janitor Closet door required adjustment to fully latch.
c. 9:40 a.m., the first floor, North Lounge door was not smoke-tight.
d. 9:45 a.m., the first floor, Patient Room 114 door was not smoke-tight.

Exit interview with the Facilities Director on October 6, 2021, between 10:45 a.m. and 11:00 a.m., confirmed the corridor opening deficiences.
