Based on observation, the facility failed to provide self-closing device on the door to the Linen Room across from the nurses station.
Findings include:
At 11:30 a.m., on 12/13/11, it was observed that the Linen Room across from the nurses station was approximately 90 sq. feet and was not equipped with a self-closing device.

Based on observation, the facility failed to provide for a properly installed, maintained and tested fire alarm system.
Findings include:
At 11:00 a.m., on 12/13/11, it was observed that the Fire Alarm Control Panel was located in the Boiler Room. This room was not a continuously occupied location. The FACP was not electronically supervised by a smoke detector.
Actual NFPA standard: In areas that are not continuously occupied, automatic smoke detection shall be provided at the location of each fire alarm control units to provide notification of fire at that location. NFPA 72 section 1-5.6
