Based on observation and interview, it was determined the facility failed to maintain the two- hour fire barrier in two instances, for two of 12 two-hour fire rated walls.
Findings include:
1. Observation on December 13, 2018, revealed the following two-hour fire barrier deficiencies above the ceiling:
a) 10:26 a.m., an unsealed conduit penetration above the cross corridor doors next to room E-839 on the eighth floor;
b) 12:31 p.m., an unsealed wire penetration above desk eight in the Starzl Clinic on the seventh floor.
Interview with the Quality Manager and U.P.M.C. Facility/Safety staff on December 13, 2018, at 2:30 p.m., confirmed the two-hour fire barrier deficiencies were present.

Based on observation and interview, it was determined the facility failed to maintain vertical openings between floors in one instance, affecting two of 13 floors.
Findings include:
1. Observation on December 13, 2018, at 1:20 p.m., revealed multiple unsealed conduit penetrations in room S-352.4 on the third floor.
Interview with the Quality Manager and UPMC Facility/Safety staff on December 13, 2018, at 2:30 p.m., confirmed the unsealed vertical penetrations were present.

Based on observation and interview, it was determined the facility failed to maintain electrical wiring in one instance, on one of 13 floors. Installation shall be in accordance with NFPA 70, National Electric Code. 19.5.1.1, NFPA 101.
Findings include:
1. Observation on December 13, 2018, at 12:37 p.m., revealed an unterminated electrical wire above the ceiling in room 639.4, Bed 6.
Interview with the Quality Manager and UPMC Facility/Safety staff on December 13, 2018, at 2:30 p.m., confirmed the unterminated electric wire.
