Doors with Self-Closing Devices

Based on observation it was revealed that the facility has failed to maintain all of the fire rated door assemblys.
Finding include;
On 4/10/23 at 11 AM, it was revealed that the double fire exit doors on both stairwells are not fully protected as fire rated doors.
The doors on both exits are missing the astrel or rated strip that join the 2 doors when closed. This strip prevents the passage of smoke into the rated stairwell when there is a fire. The stairwell is a 2 hour protected inclosure, and the doors are raated for such. All doors leading into the stairwell on each level must be corrected.
This was confirmed by the Director of Maintenance.
Emergency Lighting

Based on observation it was reveled that the facility has failed to maintain emergency lighting for exits.
Findings include;
On 3/10/23 at 11 AM, it was revealed that the battery emergency lighting on the 3rd flor , across from the nursing station, that lights up the exit route to stairs and elevator was not operational when tested.
This was confirmed by the Director of Maintenance.
