On the day of the validation survey, based on observation and staff interview, the facility failed to ensure that the interior finish for rooms was maintained in accordance with NFPA 101 19.3.3.1,19.3.3.2.

Findings include:



Observation on 09/19/12 at 12:05 PM revealed that the drywall behind the washer was damaged this was located in the laundry room. This was verified with the Director of Maintenance at the time of discovery.

On the day of the validation survey, based on observation and staff interview, the facility failed to ensure that the hazardous areas were maintained in accordance with NFPA 101 19.3.2.1. These deficient practices that would not resist the passage of smoke through out the smoke compartments could affect all residents as well as staff and visitors.

Findings include:

During the tour of the facility with the Director of Maintenance on 09-19-12 the following items were observed and confirmed by the DM at the time of discovery:

1. Observation at 10:30 AM revealed that the pharmacy storage room had open unprotected penetrations around the newly installed sprinkler pipes.
2. Observation at 12:00 PM revealed that the room across from room 20 was converted into a storage room contained combustibles and was not equipped with positive latching hardware it had a manual dead bolt that was key operated. The door would not have latched since it would have to be manually latched with a key.
3. Observation at 12:01 PM revealed that the phone room had open unprotected penetrations in the drywall from where equipment was removed.
4. Observation at 12:10 PM revealed that the housekeeping office contained combustibles and was not equipped with positive latching hardware it had a manual dead bolt that was key operated. The door would not have latched since it would have to be manually latched with a key.
5. Observation at 12:20 PM revealed that the door to the medical records office which contained combustible storage was not self closing when tested.
6. Observation at 12:35 PM revealed that the door to the kitchen was not aligned properly and would not latch when tested.
