Based on record review, observations, and staff interview, it was determined that the facility failed to ensure that the hospital was maintained to ensure the safety of patients.

Findings were:

Cross-refer to A-701 as it relates to failure of the facility to maintain all hospital systems and the physical plant;

Cross-refer to A-710 as it relates to failure of the facility to meet the Life Safety Code requirements of the National Fire Protection Association (NFPA);

Cross-refer to A-711 as it relates to failure of the facility to ensure that outside exits were provided with emergency lighting of 1 1/2 hour duration;

Cross-refer to A-716 as it relates to failure of the facility to meet the requirements for alcohol dispensers in health care facilities; and

Cross-refer to A-724 as it relates to failure of the facility to maintain the facility and equipment to provide a safe environment.

MAINTENANCE OF PHYSICAL PLANT

LIFE SAFETY FROM FIRE

Based on record review, observations, and staff interview, it was determined that the facility failed to meet the applicable provisions of the Life Safety Code of the National Fire Protection Association (NFPA), placing all patients at risk.

Findings were:
