Based on record reviews and interviews, the hospital failed to review, investigate, and resolve each patient's grievance within a reasonable time frame and provide a response to the patient at the completion of the investigation as evidenced by failure to have documented evidence that a patient's grievance resolution letter was mailed detailing the investigation and resolution.
Findings:
Review of the hospital's policy 1-4.5.0 titled, "Complaint/Grievance Process" revealed in part:
The hospital's Administrator/Assistant Administrator will be responsible for the review, investigation and resolve of all patient grievances. In its resolution of the grievance, the hospital will provide a written notice to the person filing the grievance within 10 calendar days.
Review of Patient #2's Complaint/Grievance Form revealed the administrator had received a grievance from the patient's power of attorney via email on 07/20/2021.
In an interview on 08/17/2021 at 8:00 a.m., S1Administrator acknowledged as of today, a written notice of the resolution of the grievance has not been mailed to the person filing the grievance.

Based on record review and interviews, the registered nurse responsible for supervision and evaluation of the nursing care failed to ensure that care was provided in accordance with hospital policy relative to hygiene for 3 (#2, #5, #6) patients surveyed out of a total sample of 6 patients.
Findings:
Review of hospital policy 9-2.14.0 titled, "Patient Hygiene" revealed in part:
Purpose - To ensure daily patient hygiene and comfort. Policy - Patient hygiene will be provided daily and as needed.
Routine hygiene will include in part: Complete bath or shower, as applicable daily. Oral care twice per day and as needed; oral care for NPO (nothing by mouth) patients will be at least every 4 hours.
Review of Patient #2's medical record revealed an admit date of 06/16/2021 and a discharge date of 07/22/2021. Further review failed to reveal hygiene care was performed on 06/25/2021 and 06/28/2021. Continued review failed to reveal oral care was performed per hospital policy on 06/18/2021 through 06/23/2021; 06/26/2021; 07/01/2021 through 07/08/2021 and 07/15/2021 through 07/21/2021.
Review of Patient #5's medical record revealed an admit date of 06/23/2021 and a discharge date of 07/26/2021. Further review failed to reveal patient hygiene was performed on 06/26/2021, 07/09/2021, 07/23/2021, 07/10/2021, 07/20/2021 and 07/23/2021.
Review of Patient #6's medical record revealed an admit date of 08/13/2021 and physician orders of NPO. Continued review failed to reveal oral care was completed per hospital policy on 08/14/2021, 08/15/2021, and 08/16/2021.
Interview on 08/17/2021 at 1:53 p.m. S5RN confirmed the findings for Patients #2, #5, and #6's routine hygiene was not provided in accordance with their hospital policy.
