Based on medical record review and interview the facility failed to ensure restraints were discontinued for 1 of 4 patients (#4) resulting in the potential of restraining a patient beyond need for restraint.
On 1/18/2012 at approximately 1125 during medical record review it was revealed restraint orders had been used as an as needed for patient #4 on 12/15/2011. An interview with staff #B confirmed the order had been not been discontinued and restraints had been used as needed without a new restraint order being obtained from the physician.
