Facility failed to maintain smoke barrier doors in accordance with the 2000 edition of the Life Safety Code, NFPA 101. Findings included the following:
Surveyor observed between 2:00 and 2:30pm on 02/03/13 that smoke barrier doors near nurse station between Zones 3 & 5 were not readily operable in the closed position upon release by the fire alarm in accordance with NFPA 101: 19.3.7.6.

Surveyor between 2:30 and 3:00 pm on 02/04/13 observed that double metal exit doors from corridor near housekeeping storage romm to outside was locked with slide bolt. This does not meet NFPA 101, 2000: 19.2.1.

Facility failed to maintain mechanical systems in accordance with the Standard for Installation of Air Conditioning and Ventilating Systems, NFPA 90A. Findings included the following:
Surveyor observed between 2:30 and 3:00pm on 02/04/13 that Fire Damper near shower room in Zone 3 did not close on initiation of the fire alarm in accordance with NFPA 90A 3-4.7.

Surveyor observed that between 2:00 and 2:30pm on 02/04/13 that one of two Class K fire extinguisher locations in dietary was not properly placarded to read "fire protection system shall be activated prior to using fire extinguisher" in accordance with NFPA 10: 4.3.2.2.

Surveyor observed that between 2:00 and 2:30pm on 02/04/13
1. that an electrical junction box in the Mechanical room was not covered in accordance with NFPA 70: 370-16.
2. that electrical panelboards in cooridor near nurse station in Zone 3 and Zone 5 were not properly secured in locked position in accordance with NFPA 70:110-17
