Based on observations, the facility failed to maintain the exit signage.
The finding included:
Observation on 4/16/18 at 11:58 AM, revealed the exit sign across from the main nurses station was missing the cover. The missing cover prevented the the word exit from being displayed on one side of the sign. NFPA 101, 19.2.10.1 (2012 Ed)
This finding was verified by the maintenance supervisor during the survey and was acknowledged by the administrator during the exit conference on 4/16/18.
Hazardous Areas - Enclosure

Based on observations, the facility failed to maintain the hazardous areas.
The findings included:
1. Observation on 4/16/18 at 11:55 AM, revealed the self-closing device on the IT storage room door had been disconnected.
2. Observation on 4/16/18 at 11:05 AM, revealed the purchasing department storage room door was not equipped with a self-closing device.

Portable Space Heaters

Based on observations, the facility failed to prohibit portable space heating devices.
The findings included:
Observations on 4/16/18 between 10:00 AM and 1:00 PM, revealed portable space heating devices that exceeded 212 degrees in the following locations:
a. Emergency room office
b. Respiratory therapy room
c. IT office
d. Pharmacy area
e. Doctors lounge

Electrical Equipment - Power Cords and Extens

Based on observations, the facility failed to maintain the electrical equipment.
The finding included:
Observation on 4/16/18 between 10:00 AM and 1:00 PM, revealed extension cords in the following locations:
a. Surgery dictation office
b. laboratory room
c. Pharmacy area

Gas Equipment - Precautions for Handling Oxyg

Based on observations, the failed to maintain the oxygen cylinders.
The finding included:
Observation on 4/16/18 at 10:00 AM, revealed 3 oxygen cylinders laying on the floor unsecured in the respiratory therapy bathroom.
