Facility failed to maintain hazardous areas in accordance with the 2000 edition of the Life Safety Code, NFPA 101. Findings included the following:

Surveyor observed between 1:00 and 2:00 pm on 2/13/13 that soil utility room in nursing wing did not have self closing door and in med records room double doors to adjacent space were not securely closed.

Facility did not ensure that fire alarm system was inspected monthly. Findings included the following:

Surveyor observed between 11am and 11:30 am on 2/13/13 that fire plan did not include monthly inspection of the fire alarm system in accordance with NFPA 72: 7.3-2.

Facility failed to maintain electrical receptacles in the patient care areas in accordance with the 1999 edition of the Standard for Health Care Facilities, NFPA 99. Findings included:

Surveyor observed between 1:30 and 2:00 pm on 2/13/13 that facility records did not include grounding system tests in patient care areas. Receptacles shall be tested in Patient Care areas as follows (NFPA 99: 3-3.3.3):

Physical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shall be not less than 4 oz. (test)

Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually.

Facility failed to maintain electrical systems in accordance with the Natioinal Electrical Code, NFPA 70. Findings include:

Surveyor observed that between 1:00 and 2:00 pm on 02/13/13 that electrical panelboards in various locations were not properly secured in locked position in accordance with NFPA 70:110-17
