Facility failed to maintain corridor doors in accordance with the 2000 edition of the Life Safety Code, NFPA (National Fire Protection Association) 101. Findings included the following:

Surveyor observed between the 1:00 and 2:00 pm on 2/21/13 that 2 rooms, CT room and pantry in nursing had impediments (door wedges) to closing the doors and that 2 rooms, XRay #1 and X Ray #2 had roller latches.

Facility failed to maintain hazardous rooms in accordance with the Life Safety Code, NFPA 101. Findings included the following"

Surveyor observed between 1:00 and 2:00 pm on 2/21/13 that clean linen room in nursing was storing combustibles, was greater that 50 sq ft and did not have self closing device on door.

Facility failed to maintain fire alarm in accordance with NFPA 72, Fire Alarm Code. Findings included the following:

Surveyor observed between 11:00 and 11:45 am on 2.21.13 that records did not include annual maintenance indicating compliance with NFPA 72.

Faclity failed to maintain medical gases in accordance with the Stand for Health Care Facilities, NFPA 99, 1999. Findings included the following:

Surveyor observed between 11:00 and 11:45 am on 2/21/13 that documentation,dated 7/10/12 from Stacy Systems indicated that oxygen alarm at Birthing signal at 30 psi but should be 39 psi and vacuum is not monitored. Recovery area does not have alarm.

Facilty failed to maintain electrical panelboards in accordance with the National Electrical Code, NFPA 70. Findings included the following:

Surveyor observed that between 1:00 and 2:00 pm on 02/21/13 that electrical panelboards in nursing unit corridor were not properly secured in locked position in accordance with NFPA 70:110-17
