Based on observation, the facility failed to continuously maintain the automatic sprinkler system free of deficiencies in accordance with NFPA 25.
Findings include:
Observation during documentation review, on 10/29/2015 at 9:39 AM, revealed the third party test records for the automatic sprinkler systems listed the following deficiency:
(1) The Aurora split case fire pump 481BF type 3X481X10 serial number 96-67593 has a broken packing gland eye bolt.
