Based on observations, testing and staff interview, it was determined that the facility failed to ensure that doors in vertical openings closed and latched automatically, potentially placing 25 patients at risk in case of a fire.

During a tour of this facility with staff M on 02/10/10 at 10:20 a.m., it was noted that the stairwell door to the basement mechanical room was not provided with a self closing device.

An interview with staff M on 02/10/10 at 10:20 a.m. revealed that the staff member was unaware of the deficiency.

During a tour of this facility with staff M on 02/10/10 at 12:00 p.m., it was noted that the walls between the sprinkler riser closet and a patient room and the vacuum pump room and the remainder of the building had unsealed penetrations.

An interview with staff M on 02/10/10 at 12:00 p.m. revealed that the staff member was unaware of the deficiency.
