During a tour of this facility with staff M on 02/10/10 at 10:20 a.m., it was noted that the stairwell door to the basement mechanical room was not provided with a self closing device.
An interview with staff M on 02/10/10 at 10:20 a.m. revealed that the staff member was unaware of the deficiency.

During a tour of this facility with staff M on 02/10/10 at 12:00 p.m., it was noted that the walls between the sprinkler riser closet and a patient room and the vacuum pump room and the remainder of the building had unsealed penetrations.
An interview with staff M on 02/10/10 at 12:00 p.m. revealed that the staff member was unaware of the deficiency.
