RN SUPERVISION OF NURSING CARE

Based on document review and interview, the Registered Nurse failed to supervise the care of providing patient baths for 1 of 10 medical records (MR) reviewed. (Patient #1).

Findings include:

1. Facility policy titled "L.12.07 Skin Care Protocol" last reviewed 1/2022 indicated the following: " ...3. Braden Scale Protocol: < (greater than) 16 = At Risk. > (less than) 16 = Low Risk. 4. Low Risk (> 16). A. Skin Hygiene...i. Bathe daily with mild soap, rinse, and dry thoroughly...5. At Risk (<16). A. Skin Hygiene...ii. Bathe daily with mild soap, rinse and dry thoroughly..."

2. Review of patient #1's medical record indicated the following:
(A) The patient was admitted on 2/25/22 at 12:30 p.m. and discharged on 3/25/22 at 1:30 p.m.
(B) The medical record lacked documentation of baths or patient refusing baths for 16 out of 29 days for the
following dates: 2/25/22, 3/2/22, 3/3/22, 3/5/22, 3/7/22, 3/10/22, 3/13/22, 3/14/22, 3/15/22, 3/17/22, 3/19/22, 3/20/22, 3/22/22, 3/23/22, 3/24/22 and 3/25/22.

3. During an interview with A2 (Assistant Chief Nursing Officer) on 4/21/22 at 12:30 p.m., he/she verified that per facility policy titled "L.12.07 Skin Care Protocol" last reviewed on 1/2022, patients are to be bathed/offered a bath/shower daily.

4. During an interview with A3 (Chief Operating Officer) on 4/21/22 at 4:15 p.m. he/she verified the medical record information for patient #1 and the lack of documentation of daily bath/shower and/or refusals.
