Based on record review and interviews with hospital staff, the hospital does not ensure that all orders are written for medications to ensure safe preparation and administration. One (# 8 ) of one outpatient surgical record reviewed had a medication order that was not unique to the patient in the record. Multiple orders from multiple physicians ( #'s 1 through 7 ) were written on a single medication order form increasing the potential for unsafe preparation and administration. Two different physicians's signatures were on the order for Patient #8. There was no date or time that a physician signed the order and this order sheet had a faxed stamp with the date of 01/31/12 and time of 1110. This order was noted by Staff ? at 1105 on 1/30/12.
Findings:
1. Seven physicians' standing orders for preoperative antibiotics were contained on a single order sheet with Patient 8's name on it.
2. Two of the seven physicians's standing orders had a date of 07/20/09.
3. Seven of seven physicians' standing orders had presigned signatures.
4. Hospital staff confirmed on 02/14/12 in the afternoon that physician orders are not reviewed by the hospital to assure the orders are written clearly and have the correct elements.
