SUPERVISION OF CONTRACT STAFF

Based on record review and interview the hospital failed to ensure that non-employee nursing care staff are adequately supervised as evidenced by Extended Care of Southwest Louisiana Hospital having no full time hospital employed nursing staff. All employees are employed by hospital "a" and assigned to Department 9600 of hospital "a", a cost reporting designation for Extended Care of Southwest Louisiana. Findings:
Review of a document titled "Agreement for Hospital Management Services", dated August 1, 2011 reads in part: "Facility (Extended Care of Southwest Louisiana) desires to retain Manager (hospital "a") to provide management services as set forth herein, including, but not limited to, those set forth on Attachments 1 and 2 attached hereto..."
Review of Attachment A revealed the following: "Nurse Staffing Services: (hospital "a") will provide nurse staffing services to include, but not limited to, supply of licensed and unlicensed nursing personnel used within the Facility as needed to provide appropriate care of its patients."
Review of the staffing sheet for 09/08/11 day shift revealed SF16RN was the RN Charge Nurse on duty for the day shift. Review of a hospital "a" Employee Inquiry provided by SF1Administrator revealed S16RN was an employee of Hospital "a", Department 9600.
In an interview on 09/07/11 at 8:30 a.m. with SF1Administrator he confirmed that SF2DON is an employee of Department 9600 (code for cost reporting of hospital "a") of hospital "a".
In an interview on 09/08/11 at 10:50 a.m. with SF2DON she confirmed she is employed by hospital "a".
In an interview on 09/07/11 at 8:30 a.m. with SF1Administrator he confirmed that there are approximately 40 employees, which includes all nursing personnel and nursing assistants, who fill all nursing positions at Extended Care of Southwest Louisiana. SF1Administrator confirmed there are no full time nursing personnel directly employed by Extended Care of Southwest Louisiana. He further confirmed that all employees are employed by Department 9600 (code for cost reporting of hospital "a") of hospital "a".
