Based on record review and interview, the facility failed to provide patients at the time of their admission information explaining the process for resolution of quality of care complaints, in that, 8 of 8 patients (Patient #1, #2, #3, #4, #5, #6, #7, and #8) patient rights handout did not contain the address and phone number for HHSC complaints.
Findings included
The patient's admission packet included a Patient Right's Handout. The hand out did not contain the address and phone number for HHSC complaints submission.
During an interview on 10/02/18 ending at 2:20 PM, Personnel #2 was informed of the above findings. Personnel #2 reviewed the Patient Right's information and confirmed the finding.
