PATIENT RIGHTS: CARE IN SAFE SETTING

Based on surveyor observation, review of hospital documents and medical records, and interviews with hospital staff, the hospital failed to provide patient care in a safe setting.

Findings:

1. The hospital has changed room designation and structure of rooms containing patients without plan review and approval from the Department. On the afternoon of 01/17/2012, one surveyors noted that current Patient Rooms 108 and 203 do not have handwashing and toileting facilities in the patient rooms and those facilities cannot be accessed without exiting the rooms and entering the general corridor area. These rooms are not part of the high observation/intensive care unit. This finding was reviewed and verified with Staff A and B during the survey on 01/17/2012.

2. Review of current census sheets documented Room 108 currently had a patient (Patient #10) in the room who was on contact isolation. Surveyors observations on the afternoon of 01/17/2012 did not show isolation supplies were accessible in the hallway/available without entering the patient room. Because the room does not have water and toilet access, staff cannot wash their hands if they become soiled or dispose of the patient's elimination without leaving the room.

3. On 01/17/2012 at 1400, one surveyor observed Patient Room 203. The nurse call light was on one side of the door/doorway and the oxygen was one the other, making patient access to both devices at the same time difficult.

4. Review of Rounding sheets and medical records demonstrated the hospital has utilized Room 108, since 09/22/2011, and Room 203, since 10/19/2011, for patients. Since that time, Patients #3 and 10 were placed in Room 108 in isolation. In addition to the isolation, Patient #3 required wound care. Because the room does not have water and toilet access, staff cannot wash their hands if they become soiled or dispose of the patient's elimination without leaving the room.
