Based on observation of the facility and staff interviews, the facility failed to ensure that the vertical openings- enclosures were maintained per the requirements of:
Ref: 2012 NFPA 101 sections 8.6,19.3.1.1 through 19.3.1.6
Findings include:

Observation on 08-03-17 at approximately 11:00 hrs revealed that the fire rated door leading into the stairwell had the latch bolt taped open which prevented the door from latching. This was located near room 729.
This was verified by the Maintenance Mechanic at the of discovery and the Cheif Nursing Officer during the exit interview on 08-03-17 at approximately 13:00 hrs. The mechanic removed the tape when it was discovered.

Ref: 2012 NFPA 101 sections 19.3.6.3
Findings include:

Observation on 08-03-17 revealed that:
1. At approximately 11:30 hrs revealed that the door leading into the breakroom near 789 was not latching when tested.
2. At approximately 11:50 hrs the door to the storage room in elevator E lobby did not have any door stops on the frame. This prevented the door form being able to resist the passage of smoke.
This was verified by the Maintenance Mechanic at the of discovery and the Cheif Nursing Officer during the exit interview on 08-03-17 at approximately 13:00 hrs.
