Based on interview and record review, the facility failed to ensure a registered nurse (RN) supervised and evaluated the nursing care for each patient, in that, 5 of 10 (Patient #1, #4, #5, #6, and #9) patient's records did not evidence a daily registered nurse assessment.

Findings Included:

Patient #1's record did not evidence a registered nurse assessment for 1/2817 and 1/29/17.

Patient #4's record did not evidence a registered nurse assessment for 11/30/16

Patient #5's record did not evidence a registered nurse assessment for 12/4/16 and 12/5/16

Patient #6's record did not evidence a registered nurse assessment for 12/15/16

Patient #9's record did not evidence a registered nurse assessment for 10/15/16

During an interview on 1/30/17 at 12: PM, Personnel #13 confirmed there was not any RN assessments done on Patient's #1, #4, #5, #6, and #9. There were only LVN (licensed vocational nurse) assessments completed for those days.
