MAINTENANCE OF PHYSICAL PLANT

Based on observation and interview, the hospital failed to maintain the building in a clean and orderly fashion to ensure the safety and well-being of patients, visitors, and staff.

Findings included:

1. While touring the Behavioral Health Unit on 11-8-12 with B#1 and B#2, the following was observed creating potential safety hazards for patients, visitors, and staff:
a.) At 1045 hours in Room 303, paint was observed scraped from the walls of the patient room; the drywall finish was chipped from the wall corners in the rooms; a brown dried substance was observed on the wall.
b.) At 1047 hours in Room 307, rust and calcium build-up were observed on the shower floor, shower head, and shower control knobs; the floor around the toilet was coated with a brown/rusty substance.
c.) At 1055 hours in Room 311, approximately 18 inches of baseboard was missing; approximately 2 feet of the drywall finish was chipped on the corner of the wall; paint was observed flaking from the walls; one ceiling tile was observed to be stained; the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters.
d.) At 1057 hours in Room 313, a hole, approximately 6 inches, was observed in the wall under the window; the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters.
e.) At 1059 hours in Room 315, two stained ceiling tiles were observed.
f.) At 1100 hours in Room 317, the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters
g.) At 1101 hours in Room 316, a hole, approximately 6 inches square, was observed in the shower wall.
h.) At 1101 hours in Room 314, the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters.
i.) At 1103 hours in Room 312, the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters; a hole in the drywall, approximately 9 inches long, was observed under the window.
j.) At 1104 hours in Room 310, 3 stained ceiling tiles were observed; the drywall was chipped in the drywall corner; the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters.
k.) At 1105 in Room 308, an tear, approximately 6 inches in length, was observed in the drywall; the drywall finish was chipped in the drywall corners; the shower was observed to have calcium and rust stains; the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters.
l.) At 1107 in Room 304, a tear was observed in the drywall; the door closure unit was uncovered and the electrical wires to the call light were exposed; the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters.
m.) At 1108 hours in Room 302, the door to the patient room was splintered, potentially exposing patients, visitors, and staff to hand/finger splinters.
n.) At 1107 hours in the Patient Storage Room, it was observed that one ceiling tile was missing.
o.) At 1112 in the Quiet Room One, it was observed that all the baseboard was missing.
p.) At 1115 in the Quiet Room Two, it was observed that all the baseboard was missing; the drywall tape in 2 corners had been pulled loose; the toilet was running and had copious brown stains.
q.) At 1120 hours in Room 319, a dried, brown substance was observed on the wall.
r.) At 1122 hours in the hallway outside the dining room, the baseboard was loose and caved in; the wheelchair scale had copious amounts of dirt and the rubber mat was torn and not intact.
s.) At 1155 hours in the hallway outside Materials Management Unit, 6 ceiling tiles were observed to be stained.
t.) At 1157 hours in the hallway outside the Cafe, 3 ceiling tiles were observed to be stained.
2. Interviews with B#1 and B#2 from 1045 hours through 1157 hours during the tour of the Behavioral Health Unit and the basement level of the hospital, confirmed each of the findings observed.
