Based on observations and records review made on September 30, 2011, the facility failed to maintain the auto matic sprinkler system.
Findings Include:
Records of automatic sprinkler inspections indicate that the PIV#1 and PIV #2 control valves have had the tamper switches disconnected and removed, with the control valves locked in the open position
