Based on a review of facility policies and observations and staff interviews (EMP), it was determined that the facility failed to ensure that the system developed for controlling infections and communicable diseases of patients and personnel was observed.
Findings include:
1) Review of facility policy, "Isolation Precautions, reviewed December 2014" stated, "Purpose: To reduce risk of transmission of epidemiologically important for which additional precautions beyond Standard Precautions are needed. To successfully interrupt spread from patients who are known or suspected to be infected or colonized by such microorganisms to other patients and healthcare workers. To prevent the spread of microorganisms among patients, personnel and visitors. ... . 2. Droplet Precautions: for agents that are transmitted through contact of the conjunctivae or the mucous membranes of the nose or mouth of a susceptible person with large-particle droplets. ... . c. Mask: Wear a surgical mask when entering the room . ... ."
2) During a tour of the High Observation Unit on June 11, 2015, room 3011 was noted to have a Droplet Precautions sign posted. Upon entering room 3011, it was noted that EMP12 was not wearing a mask.
3) During an interview on June 11, 2015, EMP8 confirmed the above and stated, "[EMP12] went in to the room because the patient was restless and he was afraid the patient would fall out of bed."
