MAINTENANCE OF PHYSICAL PLANT

Based on observation and interview with facility staff, the facility failed to maintain the physical plant and environment in such a manner that the safety and well-being of patients are assured as evidenced by damaged electrical outlet covers and pooling condensation around electrical outlets. There were also multiple water-stained ceiling tiles.

The findings were:

During a tour of the facility on 5/18/21, it was observed:
Room #1 (Negative Pressure Room)
This room was unoccupied. The surveyor inspected the electrical outlet covers by the head of the patient bed. Three of the electrical outlet covers were bent or loose and heavy dust particles/lint noted around the visible opening. It was also noted that two electrical outlet covers had clear liquid condensation on the bottom edge of the covers.

In an interview with Staff #1, Staff#2, and Staff #3 on the afternoon of 5/18/21, all three acknowledged the findings above.
