Based on observation and interview it was determined the facility failed to maintain one two-hour common wall affecting one of two levels in this component.
Findings include:
1. Observation on December 12, 2016, at 11:18 AM, revealed an unprotected horizontal penetration around a telephone cable in the number 9 mechanical room portion of the two-hour fire rated common wall.
Exit interview with facility maintenance representatives #1 and #2 on December 12, 2016, between 11:45 AM and 12:00 PM, confirmed the unprotected penetration in the basement common wall.
Corridor - Doors

Based on observation and interview it was determined the facility failed to maintain one of aver 37 corridor doors in this component.
Findings include:
1. Observation on December 12, 2016, at 10:27 AM, revealed the first floor IT room door, located across from the Med/Surg nurses station, was blocked open by a spool of communications wire.
Exit interview with facility maintenance representatives #1 and #2 on December 12, 2016, between 11:45 AM and 12:00 PM, confirmed the IT room door was blocked open.
