Based on document review, the registered nurse failed to implement additional pain intervention or contact patient provider after ineffective initial pain intervention in 1 of 10 patient medical records (MR) reviewed (P5).

Findings include:

1. Review of policy/procedure "Pain Management," last approved 02/08/2022, indicates the following:

C. Reassess and respond to patient's pain plan throughout RN's shift.
5. If current pain management plan not effective, consider implementation of additional interventions if appropriate or contact patient's provider.

2. MR indicated P5's initial pain assessment was completed at 12:10 p.m., patient rated their pain 9/10. They were given pain medication at 1:12 p.m. P5 pain reassessment indicated pain level 8/10. No further pain interventions or physician notification regarding ineffective pain management were completed.
