Based on observations made during review of the logs, policies and procedures of the facility between the hours of 1:30 pm and 5:30 pm, while accompanied by the Director Of Plant Ops, Manager Of Plant Ops, Director Of Nursing, Safety Officer, Educator / Infection Control, and Senior Maintenance Mechanic the facility failed to provide logs documenting that fire alarm system was tested monthly by staff as required.
