Based on observations and interview, the facility failed to maintain 3 smoke barriers free of penetrations. This affects 4 of 7 smoke zones, affecting approximately 14 patients and 7 staff members. The facility had a capacity of 50 patients and a census of 26 patients.
Findings include:
Observations and interview on 5-24-13 revealed the following:
1. A hole (approximately 1/4 inches in size) located above the suspended ceiling in the Service Hall smoke barrier.
2. A hole (approximately 3 inches by 3 inches in size) located above the suspended ceiling, in the smoke barrier that is located between Rooms 2 and 4.
3. A hole (approximately 1 inch in size) located above the suspended ceiling in the smoke barrier at the Surgery Entrance.
Maintenance Staff A verified these observations at the time of the survey process.
