Based on review of records and interview the hospital failed to administer medication to 1 of 1 patient, (Patient #1). Patient #1 did not recieve the medication in accordance with Federal and State laws, the orders of the practioner and accepted standards of the practitioner or practitioners responsible for the patient's care as specified under 482.12(c), and accepted standards of practice.

Findings include

During record review of the Medical Administration Record dated 06/21/2016 reflected, Patient #1 did not receive scheduled Armour Thyroid 30 mg medication first dose on 06/21/16 at 0800. The record revealed it was not available.

During record review of the Medication Administration Record dated 06/21/16 reflected, Patient #1 did not receive scheduled Bystolic 5 mg on 06/21/16 at 2100.

During record review the hospital Physician Orders and Directions on 06/20/16 at 1300 reflected, "Pravastiatin 80 mg daily, Armour Thyroid 30 mg daily, Bystolic 5 mg daily ..."

During a face to face interview with Personnel #4, reported that she received home medications for the patient on 06/22/16 and approved Thyroid Armour .25 mg Bystolic Tabs 5 mg, Progesterone Capsules 50 mg, Vitamin D3 5000, and Pravastatin 80mg. These medications were signed off and approved on 06/23/16 at 1500.

During a face to face interview with Personnel #1, verified the records did not reflect that the patient received her medication as written in the physician's order or the documented time slots on the MAR. The patient did not receive the medication.

Policy
The hospital Pharmacy Hold Orders Policy dated 11/24/14 reflected, "To provide a uniform procedure for the ordering, dispensing, administration and documentation of hold orders written by prescribers."
