Based on observation and record review it was determined the facility failed to ensure their medical gas systems were inspected annually as required. Findings:
On 06/01/2016 at 11:04 a.m., one detailed inspection report from Airgas, LLC dated 05/21/2016 was reviewed and no issues were identified with the facility medical gas systems. A cover letter dated 05/26/2016 from Airgas USA was reviewed and in part stated there were no problems or issues with the facility medical gas systems. The installation report of the facility medical gas systems dated June 1, 2009 identified the facility medical gas system as a Level 2 system.
On 06/1/2016 at 11:07 a.m., an interview was conducted with the administrator of the facility. The administrator was asked why the facility only had one medical gas inspection reports for the facility. The administrator advised she did not know. She said that Airgas performed an inspection of their medical gas systems on 5/21/2016 and they received the final report on 5/26/2016 which did not identify any problems.
