Based on file review and interview, the facility failed to document orientation and up-dated education for 2 (#J, #K) of 2 contingent and/or agency nursing staff, out of a total sample of 7, resulting in the potential for staff not following facility policies/procedures and documentation requirements. Findings include:

Interview with the Director of Nursing (DON), on 7/28/15 at approximately 1530, revealed that most of the facility nursing staff had been contingent, part-time or agency staff, but that was changing. Review of nursing personnel files, on 7/29/15 at 1230, revealed that Nurse #J was contingent and #K was an agency nurse, both continued to work at the facility. Contingent Nurse #J did not have facility orientation on file that included documentation requirements and Agency Nurse #K did not have documentation of facility orientation on documentation or the updated 6/2/15 required documentation training for baths, turning, and skin/wound care. Interview with the DON and CEO (Chief Executive Officer), on 7/29/15 at approximately 1315, verified that this training was not documented on those two individuals.
