Based on record review and interview, the facility's governing body did not ensure that the contracted radiology furnished services according to the guidelines agreed upon by the hospital and contractor, citing 1 of 1 patient who had a "STAT" order for chest x-ray on 11/14/15. The contracted radiology arrived at the facility 4 hours and 13 minutes after the call was placed.

Findings included:

Patient #1 was admitted on 11/13/15 at 7:20 PM for mobility and ADL deficits (activities of daily living). On 11/14/15 a physician ordered a "STAT" order for chest x-ray. Personnel #3 called in the order to the contracted radiology on 11/14/15 at 11:17 AM. The contracted radiology was in the facility at 3:30 PM to complete the "STAT" chest x-ray, 4 hours and 13 minutes after it was called in.

The contracted radiology "Turnaround Times" undated reflected "X-ray: STATS - Technician will arrive 2 hours and results within 4 hours from the placement of the call."

In an interview on 3/16/16 at approximately 1:00 PM, Personnel #1 was informed of the findings and she confirmed the findings.

Based on interview and record review, the facility's registered nurse did not supervise and evaluate the nursing care for 1 of 1 patient who complained of pain (pain scale of 8 out of 10) on 11/13/15 at 9:30 PM and was given pain medication on 11/14/15 at 12:10 AM, 2 hours and 40 minutes later.

Findings included:

Patient #1 was admitted on 11/13/15 at 7:20 PM for mobility and ADL deficits (activities of daily living). At 9:30 PM Patient #1 complained of back pain to Personnel #6. Personnel #6 noted the initial pain score was 8. On 11/14/15 at 12:10 AM, Personnel #6 medicated Patient #1 with "Percocet 10/325 milligram 2 tablets," 2 hours and 40 minutes after the patient initially complained of pain.

In an interview on 3/21/16 at approximately 9:00 AM, Personnel #1 was informed of the above findings. She confirmed the findings.
