On the day of the recertification survey based on direct visual observations of the entire facility from 10:00 a.m. to 4:00 p.m., the facility failed to ensure that the electrical wiring and equipment were being maintained in accordance with code.

Findings Include:

1. The Advance vacuum in the custodial room across from room 12 was missing its ground pin on the electrical plug.

2. The case manager's office had a six outlet multi-plug adaptor with a refrigerator plugged into it.

3. A large relocatable power tap was mounted on the corridor wall by room 8.

4. The patient access services office had a toaster plugged into an extension cord and a refrigerator plugged into a heavy duty extension cord that was run through a doorway.

5. The respiratory office had a relocatable power tap with a coffee pot and a microwave plugged into it.
